All information about data controller is written down in the below. In order to get rid of so many repeats the name of the data controller is shortened. In the following sections of this information notice the data controller is named as indicated in the box.

Due to the obligations resulting from law numbered 6698, called Personal Data Protection Law and General Data Protection Regulation and Turkish Law in general, we Özbudent, as a data controller, process the personal data of patients lawfully.

INFORMATION

We, Özbudent Diş Hastaneleri, as a data controller, take into consider as a vital issue processing the personal data of our patients lawfully and protecting the privacy of our patients. By this information notice, we would like to inform our patients about our data processing activities in detail. So that this information notice is written down in compliance with the mandatory requirements listed in the article 10 of Personal Data Protection Law and provisions of General Data Protection Regulation (GDPR) in general.

We, Özbudent Diş Hastaneleri, consider the right to privacy and protection of personal data of our patients at an upmost significance. Therefore, we would like to inform our patients about our data processing activities and policies for the sake of right to privacy.

LOCATION	Execution of Operations Lawfully, Finance and Accounting Operations, Legal Operations.	Necessary for the Establishment, Exercise or Protection of any Right.
CUSTOMER OPERATIONS	Finance and Accounting Operations, Legal Operations.	Necessary for the Establishment, Exercise or Protection of any Right.
HEALTH DATA	Execution of Sales of Services, Execution of Processes of Service Production, Performance of Contracts.	Data in relation to health and sex life can be processed only for the following purposes; protection of public health, pretentive medicine, diagnosis, execution of treatment operations, planning of healthcare services and its finance projects, by people who are obliged to keep secret due to professional ethics or authorized legal persons and instutions without providing the consent of data subject.

INFORMATION ABOUT DATA TRANSFER

We, Özbudent Diş Hastaneleri, according to Personal Data Protection Law and GDPR, may transfer your personal data to the following ones for the explained purposes in parenthesis;

1. Attorney at Law, (For the purpose of Execution of Legal Requirements and limited to that purpose)

 

2. Accountant, (For the purpose of Execution of Finance and Accounting and limited to that purpose)

 

3. Electronic- Invoice Integrator and Preservation System, (For the purpose of Execution of Finance and Accounting business and limited to that purpose)

 

4. Authorized Public Foundations (In case when information is demanded from government officials especially from Ministry of Treasury and Finance and Revenue Administration, Execution of Business Operations lawfully and limited to those purposes),

 

5. Third parties from digital media and social media accounts for the purpose of advertisement (In case when data subject gives consent to that purposes and limited to those purposes),

Personal data of our patients is never transferred to abroad.

4. MEANS FOR COLLECTION OF PERSONAL DATA

 

Your personal data may be collected in

 

1. Physical Environment (with non-automated means) such as in verbal, contracts signed in our clinics, consent forms, patient registration documents, invoices, call center records, voucher, order information, demand information, printed forms and other official documents that you sent to us,

 

1. Online Environment (with automated means) such as; e-mails and online communication form via our website.

 

5. RIGHTS IN RELATION TO PROTECTION OF PERSONAL DATA

 

1. Having information about whether your personal data is processed or not,
2. Demanding information about it in case when your personal data is processed,
3. Having information about the purposes for your personal data to be processed and whether your processed data is used properly to its processing purpose,
4. Having information about the third parties, who are domestic or in abroad, that your personal data is transferred.
5. Demanding revision when your personal data is processed wrongfully or missing, and also demanding that to the third parties that your data is transferred is being informed about this revision.
6. Even though processed lawfully, if the reasons for processing is no longer available you can demand the deletion and disposal of your personal data and also demanding that to the third parties that your data is transferred is being informed about this deletion and disposal.
7. If your processed data is analyzed exclusively by automatic systems and if there is an opposite result to you, you can make an objection.
8. Demanding your damages to be compensated if those damages arise due to the unlawful processing of personal data.

If those aforementioned rights are exercised by the data subject, he or she will be informed by the communication data which he or she given to us.
